Each record includes facility ID, name, full address, phone, hospital type (Acute Care / Critical Access / Psychiatric / etc.), ownership category, emergency services flag, birthing-friendly designation, CMS overall star rating, and per-measure-group counts (mortality, safety, readmission, patient experience, effectiveness, timeliness, medical imaging).
